Application for Various Certificates

For current students

The following certificates are available from the automatic certificate machine.

・Student registration certificate
・Academic transcripts
・Prospective course completion certificates
・Student travel fare discount certificates
・Health certificates

For graduates

Applications are accepted directly at the counter and also by post.

Number of days required

Certificates of course completion and academic transcripts will be ready the day after an application is approved. Certificates in English will be ready 3 days after an application is approved (excluding weekends and holidays).

How to apply by post

Write "Certificate Application" on the envelope, and include the following information in the enclosed letter.

- （1）Name (include alphabetic spelling if you require certificates in English)
- （2）Graduate school and department
- （3）Month and year of university entry and graduation
- （4）Birth date
- （5）Contact telephone number
- （6）Type and number of certificates required
- （7）To be securely sealed or not (if so, please note the sealing method below)
- （8）Purpose of use and recipient's name

Return-mail envelope (return addressee noted, stamps attached)
